**Important Notice Regarding Availability of Forms 1095-B and 1095-C**
In accordance with the reporting requirements of the Affordable Care Act (ACA), Form 1095-B and Form 1095-C are available upon request.
A responsible individual or full-time employee may request a copy of their Form 1095-B or Form 1095-C by contacting us using one of the methods listed below:

You may request a copy of your form, which becomes available in March, by any method above.
Please include the your full name and mailing address, and include additional identifying information necessary for us to locate the appropriate form.
A copy of the requested Form 1095-B or Form 1095-C will be provided within 30 days of the date the request is received or by January 31 of the following calendar year, whichever is earlier.
