Island Trees School District

Registration Please contact the District Registrar for all new registrations. Children are eligible to start Kindergarten if they turn 5 years of age on or before December 1st of that year. At the time of registration Parents/ Guardians must present the child’s birth certificate or other official proof of age. Proof of immunizations and proof of Island Trees School District residency. We are open all summer.

 

MINIMUM REQUIREMENT FOR SCHOOL ATTENDANCE:

• DTaP, DTP: 4-5 doses required (final dose after age 4)

• Tdap: required for students grades 6-12, must be received by age 11

• POLIO (IPV/OPV): 3-4 doses required( final dose after 4yrs)

• MMR: 2 doses required for all grades

• HEPATITIS B: 3 dose series required

• CHICKEN POX: 2 doses required for Kindergarten, all grades

• Meningitis: 1 dose required in 7th and 12th grade. (new for 2016)
